To decommission a laboratory previously approved for radioisotope use, please complete the appropriate section of the authorization amendment request form and submit that form to the Radiation Safety section (Campus Mail Box 980112). A member of the Radiation Safety staff will conduct a close-out survey of the laboratory which includes a complete swipe/geiger counter survey and removal of all postings, including caution signs and labels. Please insure that all radioactive material is removed from the laboratory before requesting a close-out survey.
