Ways to Make a Gift
| There are many ways you can make a gift to your alma mater, the Department
of Occupational Therapy. |

Send a Check made out to MCVF/OCCT. Mail this check to the Department of
Occupational Therapy, VCU c/o Dr. Shelly J. Lane, P.O. Box 980008, Richmond, VA 23298-0008. |

Make a Credit Card Gift on-line. Visit www.vcu.edu/giving/ |
| Step 1: designate a gift amount, select a payment option, and select School
of Allied Health in the purpose section. Then, in the block under purpose that says "specify", indicate one of the
following: |

Occupational Therapy (if you do not have a specific fund in mind); or |
| - |
Occupational Therapy: Cynthia Gudger-Garris Scholarship; or |
| - |
Occupational Therapy: Patti A. Maurer Scholarship; or |
| - |
Occupational Therapy: C.A. Kooiman Memorial Award |
| This last part under "specify" is VITAL. Without such designations, the money may end
up staying at the Department’s parent school level, rather than making it directly to the Department and the specific
Department fund you’d like to contribute to. |
| Step 2: provide name on credit card, select credit card type, type in credit card
number, select credit card expiration month and year. |
| Step 3: complete at least the required name and address basic information. |
| Step 4: receive thank you and click on the exit button. This will also provide an
initial receipt for tax purposes. Another recognition of your gift will be sent to your email. |
